Evaluation & Treatment Services
Bloom Wildflowers Therapy is private-pay/self-pay only at this time.* Please contact for current rates.
Being a private pay business allows me to offer discounted rates as well as treat anyone that could benefit from services. Some children do not qualify for SLP services due to insurance guidelines or standard score requirements set by school districts. These rigid measures may easily miss the full picture of not only what a child needs but why they are struggling.

*On request I am able to provide a superbill, which you can submit to your insurance company to obtain reimbursement. Click here for more information on superbills.
Payments can be made by debit card, credit card or HSA/flex-spending cards.

Payments are due at the end of each visit.
Evaluation Services
First Consultation - Free
Not sure if your child needs services? Want to know more about teletherapy? Have specific concerns you want to discuss before scheduling an evaluation?
I offer a free 20 minute consultation via video conferencing to discuss your concerns and show you how online therapy can work for your child.

Articulation Evaluation
This is an evaluation of articulation/phonological processing or apraxia. This does not include a language assessment. The cost includes the time to gather a case history, evaluate the child and write a thorough report. This will also include a 20 minute, 1:1 session with the parents/caregivers to discuss evaluation results, diagnosis, and recommendations after the assessment and report have been completed.

Stuttering Evaluation
This is an evaluation for stuttering. This does not include a language assessment. The cost includes the time it takes to gather a case history, evaluate the child, write a report, and also includes a 20 minute, 1:1 session with the parents/caregivers to discuss evaluation results, diagnosis, and recommendations.

Comprehensive Evaluation
This is an evaluation of receptive, expressive language skills, executive functions as well as articulation, stuttering or pragmatics (as needed). The cost includes the time it takes to gather a case history, evaluate the child, write a report, and also includes a 20 minute, 1:1 session with the parents/caregivers to discuss evaluation results, diagnosis, and recommendations.

Adult Evaluations
If you are over the age of 18, contact us about evaluation options and pricing. Evaluations and services are meant to be functional and services vary depending on the needs of the individual. The goal is to provide services to maximize quality of life, whether you are a person with ADHD looking for short term coaching, a college student in need to extra support or someone who is recovering from a TBI looking for help with getting back to work. All evaluations will include a 20 minute, 1:1 session with the client to discuss evaluation results and recommendations.

Teletherapy Treatment Sessions
Therapy Sessions
Therapy can be offered for 30 minutes, 45 minutes or 60 minutes depending on the needs of the client.

Coaching and Support Services
Parent, caregiver and ADHD coaching is also available for a set amount of sessions or as an ongoing service. These sessions can be offered for 30 minutes, 40 minutes, 50 minutes or 60 minutes as well. These services would not require a traditional evaluation.

During the initial evaluation review, the SLP and parents/caregivers will discuss recommendations for the duration and frequency of services. The length and frequency of the sessions depends on what works best for the client and their families, generally it is recommended to have services 2 to 3 times a week minimum.

For direct treatment sessions, 5 minutes of each session will be used to discuss the progress in therapy and answer any questions from the caregiver. Homework may be given if the child is currently at the level of treatment that homework is beneficial.

Consultation Services
Consulting services may include but are not limited to services such as attending IEP meetings of clients, consulting with other providers when requested by caregivers, community outings or consulting with school districts to maximize learning outcomes using an executive function framework. Consulting fees will always be discussed ahead of time. Services more than 60 minutes will be billed in 30 minute increments.
Good Faith Estimate
Beginning January 1, 2022, if you’re uninsured or if you opt for self-pay (i.e., not going through your health insurance), health care providers such as myself must provide you with an estimate of the expected charges before you get an item or service. This is called a good faith estimate (GFE).
Providers and facilities (like mine) must provide you with a GFE if you request one, or after you’ve scheduled a service. It should include the expected charges for all items or services that are provided as part of the same scheduled experience. The provider must provide a list of all items and services associated with your care.

Getting Started

-
Consultation - We begin with a free initial 20 minute consultation video call to discuss your concerns. At that time we can schedule an evaluation based on your child’s areas of need if necessary.
-
Evaluation - This will consist of standardized and dynamic assessments, clinical observations, and parent and child interviews/feedback. Once completed, a summary of the findings and recommendations will be presented to the parents. This will take place at a separate consult session after all tests have been scored, analyzed and a report has been written. This allows me to get a full picture of the areas of strength and areas for growth. I will present my recommended goals and therapy plan based on the evaluation results. However the best therapy involves collaboration so the parents, caregivers and clients also are encouraged to give feedback and input about their goals for treatment.
-
Treatment - Using the goals, I create an individualized program focusing on their specific areas of need as well as areas of interests. After each session, parents/caregivers will be provided with an overview of the session along with home practice materials when the child is ready for homework.

Superbill 101
What is a superbill?
A superbill is detailed document which allows clients to bill their insurance company directly. With a superbill, the insurance company assumes the client has already paid and that any payment is therefore a reimbursement.

Finding out about coverage with superbills
-
Call your insurance company and explain you are scheduling an assessment with an out-of-network speech language pathologist. They may need a 'CPT code'. This will depend on the type of evaluation you are requesting for you child. During our initial consultation, exact codes can be discussed depending on the concerns. Common evaluation codes include:
-
Speech Sound Evaluation CPT Code 92522
-
Speech and Language Evaluation CPT Code 92523
-
Fluency (Stuttering) Evaluation CPT Code 92521
-
-
Ask your insurance what is needed in order to seek reimbursement. Questions such as:
-
Is a doctor's referral is required?
-
Do they require standardized testing?
-
Do they require a copy of the evaluation report?
-
-
At this time you also want to ask if telepractice services are covered, as this continues to vary.
-
If your insurance company needs to know what I suspect is the disorder or ICD -10 code, this can also be discussed at the consultation. Common diagnoses are phonological disorder, receptive/expressive language disorder and fluency disorder.
-
If your insurance company asks why you are seeking services from an out-of-network provider you can tell them whatever applies to your situation, for example: lack of services in the area, desire to see a specialist or perhaps you previously tried to be seen by a SLP but were denied coverage. It can also be helpful to explain how the current issues your child is experiencing are negatively impacting their quality of life.
-
Additionally if therapy data is needed when you seek reimbursement, you must inform the SLP ahead of time so that copies of the treatment notes can be provided.
